CLAREMONT CARE SERVICES
Claremont Care aims to support participants living with a disability to lead fulfilling lives. Our experienced staff listens to each participants’ preferences to ensure we deliver care with empowerment and dignity while maximising each participants’ independence. We will support each participant in his/her choices and provide personalised health and wellbeing support services to help each participant live more independently.Claremont Care’s Registered Nurses conduct and oversight all assessments, individual support plans, and reviews to ensure that each participant’s choices, aspects of health, are considered when developing individual plans and services.
Claremont Care applies a person-centered approach and delivers community-based disability support services under the following categories.
IN HOME SUPPORT SERVICES
Claremont Care ‘In home support services’, support participants with:
- maintaining home environment;
- maintaining yard environment;
- providing assistance with personal daily tasks;
- providing assistance with administering medication;
- providing community nursing care (Health Supports).

COMMUNITY ACCESS SERVICES
Claremont Care ‘Community Access Services’, support participants to engage in community, social and recreational activities. Services include:
- Transport and support to appointments in the community;
- Attend community/social and recreational activities groups;
- Innovative Community Participation.

SUPPORT COORDINATION SERVICES
Claremont Care ‘Support Coordination Services’, support participants with:
- Level 1: Coordination of Supports;
- Level 2: Coordination of Supports:
- Psychosocial Recovery Coaching.

Claremont Care ‘Respite Service’, support participants with:
- Short Term Accommodation and Assistance (Respite care);

CAPACITY BUILDING SERVICES
Claremont Care ‘Capacity Building Services’, support participants with:
- Self-Management Capacity Building;
- Capacity Building and Training in Plan and Financial Management;
- Development of Daily Living and Life Skills;
- Individual Social Skills Development;
- Other Supports – Training for Carers/Parents;
- Other Supports – Skill Development and Training to increase independence, including Public Transport Training.
